Per our Terms of Service, please do not include protected health information (PHI) — such as diagnosis details or medical history — in any web form; our team will follow up by phone to discuss medical details.

By submitting your phone number through our website forms, you consent to receive communications from our office related to your care, appointments, and requested services.

This Privacy Policy covers information collected through this website — it is not our HIPAA Notice of Privacy Practices. Protected health information (PHI) created or received in the course of your care — your exam findings, treatment records, and billing records — is governed separately by the Health Insurance Portability and Accountability Act (HIPAA) and our Notice of Privacy Practices, which is available at our office and upon request. As noted above, please do not submit PHI through this website's forms; our team follows up by phone for any medical or billing detail that requires it.

For patients using Medicare benefits, our billing follows applicable Medicare billing regulations for primary spine care services; our front desk can review Medicare billing questions specific to your case before your visit.

If you provide your mobile phone number and opt in to SMS communications through our website forms, appointment requests, or other communication methods, you consent to receive text messages from Kent Kangley Chiropractic, which may include appointment reminders, scheduling confirmations, office notifications, and follow-up or customer service messages.
